Please contact Jackie Scott, Adaptive Recreation
Specialist, to set up an interview and fill out a participant information sheet (includes
emergency information, medical references and information on disability as it effects
participation in the program) prior to participation. At the interview, a copy of
the Adaptive Recreation Guidelines for program activities will be provided for parents and
guardians during the initial interview and with new participants. Parents/guardians
and participants are required to follow the guidelines of our program. We welcome
your input into our program and invite you to join us at our Adaptive Recreation Advisory
Committee meetings or write/call us anytime at (626) 744-7257 with your suggestions and
input.
