MAP Executive Summary The MAP Campaign is a community-wide initiative to create positive change in community health. Spearheaded by the Pasadena Public Health Department, MAP is dedicated to helping the Pasadena area identify important community health issues, secure resources for addressing them, develop a plan and take action. This community health improvement model was developed by the Centers for Disease Control and Prevention, in collaboration with the National Association for City and County Health Officials. An important belief of the campaign is that genuine quality of life and good health can only be achieved by all when community members come together to effectively identify and work towards common goals. To this end, the MAP Campaign has involved faith institutions, non-profit organizations, healthcare providers, schools, youth, businesses and government officials in creating a long-term action plan to improve community health. The Campaign is guided by a core group of community leaders dedicated to making real changes in Pasadena. In order to identify and address health challenges of greatest interest to the Pasadena community, the MAP Campaign has centered in on four approaches:
The information gathered through these core Campaign elements has helped to define the focus and direction for the initiative in the coming years. In order to improve health and wellness in the Pasadena area, MAP priority areas will focus on: ¨ Ensuring greater access to health and social services. ¨ Advocating for the adoption of healthier behaviors and lifestyles. ¨ Increasing opportunities for education and training in healthcare and public health.
¨ Providing overall support for families, youth and communities as they lead healthier, more fulfilling lives.
MAP’s vision is that the greater Pasadena area will become a healthy, thriving community that values and promotes overall wellness. With the help, support and participation of all community partners, this vision can and will become a reality.
