COVID Vaccination Community Outreach Program
Saturday April 10th, 8:00 am – 4:30 pm
ChapCare Pasadena and MEMAH are partnering with Kaiser Permanente in a community vaccination outreach effort. Our goal is to increase the number of COVID-19 vaccines administered in our local community.
This outreach effort is limited to clients and family members of the partners who live in the greater San Gabriel Valley area and meet the current open COVID Vaccine eligible categories.
To be eligible in addition to the above the individual must:
Reside/work in the greater San Gabriel Valley area and be age 50 or older.
Have had no immunizations of any kind in the past 14 days prior to April 10, 2021.
Appointment Process
Kaiser will be administering the Johnson & Johnson vaccine and only one immunization is required.
To sign up eligible individuals for this community outreach vaccination event, please contact your identified outreach coordinator, Jim Morris via phone or email.
The outreach coordinator will ensure that your information is in the KP health system. The community member will need to show up with a basic form of ID at Kaiser Permanente 3280 E Foothill Blvd Pasadena, CA 91107 at least 15 mins before the appointment time.
Please advise your coordinator of any cancellations within 24 hours prior to the vaccination date, so we can inform the KP staff.
