The Pasadena Public Health Department evaluates, collects, and interprets data to reveal health conditions, disease trends, and best practice interventions that impact the Pasadena community. A Community Health Needs Assessment (CHNA) is a report that describes findings from a systematic, collaborative data collection process to gain insight into the health status and needs of the residents in a jurisdiction. Data presented in CHNAs span a wide range of topics related to community well-being, including disease rates, risk factors for disease and death, health behaviors, and social determinants of health. This examination of key health indicators in the Greater Pasadena area is designed to offer some understanding of health needs in this community and guide community health improvement planning efforts. Our CHNA is intended to provide a gateway for concerned community members, civic leaders, non-profit organizations, and policy makers to further delineate and address health issues in Greater Pasadena.
Pasadena has a long history of publishing CHNAs. Pasadena issued its flagship CHNA, the Quality of Life Index,in 1992 as a charter city for the California Healthy Cities Project and has continued since that tradition with the 1998, 2002, 2011 and 2012 editions. In 2016, the City of Pasadena Public Health Department partnered with Huntington Hospital to produce their first joint CHNA.
This CHNA report can be used as a resource for health advocates and organizations that use data to guide planning, policy development, and procurement or allocation of resources. A Community Health Improvement Plan (CHIP) is a long-term, systematic collaboration between the health department and community partners to address public health problems. A CHIP utilizes findings from the CHA to begin a dialogue between the public health department, community partners, and community members about how to improve the overall health and well-being of the city of Pasadena. The CHIP addresses more than the roles and responsibilities of the Public Health Department alone; it is also a comprehensive record of joint efforts undertaken to redress Pasadena’s most immediate health concerns. A CHIP reflects the results of a participatory planning process that includes significant involvement from a variety of community members. The intent of this plan is to promote partnerships to set priorities and coordinate and target resources to improve the health of our residents.
